Fees & Forms
Rates: I charge $175 per 55 minute session, and offer sliding scale fees.
Insurance accepted: United, Optum, UMR, GEHA, Premera, Anthem, Kaiser Permanente.
Payment: Check, Visa, Master Card, American Express, and Discover cards are all accepted for payment. If we're meeting face to face, I will also accept cash and can provide you with a receipt.
Cancellation Policy: I ask that you let me know as soon as possible if you need to reschedule or pay the full cost of the session if it's under 24 hours notice.
Intake paperwork: Once we've scheduled our first appointment I will send you paperwork to complete a week in advance of that appointment. These forms will include an intake form, disclosure form, consent forms, practice policies and a notice of privacy practices.
